Judgment
PRIOR TO FILING WITH THE PROTHONOTARY OFFICE, PLEASE REFER TO THE PA RULES OF CIVIL PROCEDURE AND LANCASTER LOCAL RULES OF COURT FOR APPROPRIATE DOCUMENTS AND RULES.

Default
File a praecipe for judgment, a dated copy of the 10 day notice, and a Pennsylvania Rule 236 form. If the debtor is an individual, you must file a notarized affidavit of nonmilitary service. File an original and a copy for each debtor with a Rule 236 form. Include a stamped envelope addressed to each debtor. If you need a timed stamped copy for your records, please send a self-addressed stamped envelope large enough and with enough postage for its return. No cover sheets are required.
Magisterial District Judge
File a certified copy of the judgment from the MDJ for each debtor and a Rule 236 form. File the original and a copy with a Rule 236 form for each debtor. Also include a local civil cover sheet with names and addresses or plaintiff and defendant. An original and copy for all parties, and a self-addressed stamped envelope large enough to hold documents with sufficient postage is required for all parties. Only Local Cover Sheet is required.
Complaint & Confession
File the complaint in compliance with Rule 2952, verification, the confession of judgment, a copy of the judgment document showing the debtor's signature, and a Rule 236 form. File an original and a copy with a Rule 236 form on top for each debtor. Include a local civil cover sheet, copies for each defendant, and an addressed, stamped envelope (correct size) for each debtor. To file for money and possession, file the complaint and two confessions. Please make checks out for correct amount according to the fee bill. Only Local Cover Sheet is required.
Foreign / Out of State
File a praecipe for judgment and certified copies of the docket entry stating favor of judgment for and against with the money amount certified, certificate of addresses, Rule 236 form, and a notarized affidavit of validity. File the original. Include a complete copy of the judgment with a Rule 236 form on top for each debtor. File an original and copy for all parties and a self-addressed stamped envelope large enough to hold documents with sufficient postage for all parties. Only Local Cover Sheet is required.
Transfer to Another State or County
Request an exemplified copy of the docket entry with money amount certified. Include a self-addressed stamped envelope with sufficient postage. Please specify if the judgment is to be transferred to another Pennsylvania County or State for recording. No cover sheets are required.
Transfer of a Judgment From Another Pennsylvania County
File a certified copy of the docket entries that include a certification of the amount of the judgment and Rule 236 form. Include a complete copy of the judgment with a Rule 236 form for each debtor. File an original and copy for all parties and a self-addressed stamped envelope large enough to hold documents with sufficient postage for all parties. No cover sheets are required.
